Hello nvs,
There seems to be an issue with the access rights of the directory, so I suggest you start the investigation by checking the permission settings of the "C:\inetpub\" folder. Furthermore, please make sure that the directory is excluded from anti-virus filtering, as anti-virus agents may temporary lock the .eml file and cause various issues.

@Daniel Novak (Vamsoft): Yes, this is permission-related issue (domain account used for ORF service). Thank you.
On Arrival error: Unexpected exception EEngineError "Error writing email to the file system (C:\inetpub\mailroot\Pickup\ORFMAIL-2ECA128D2DCFBCB295A2FEB09CD34726.eml): Cannot create file "C:\inetpub\mailroot\Pickup\ORFMAIL-2ECA128D2DCFBCB295A2FEB09CD34726.eml". Access is denied".
